Good Faith Estimate

A clearer estimate before care begins.

If you do not have insurance or choose not to use it, you may have a right to a written estimate of expected charges.

Acuity Counseling delivers personalized Good Faith Estimates through the secure client portal. This page explains the process, but it is not your formal estimate.

When it applies

Know when an estimate may be available.

Federal protections generally apply when you are uninsured or are not using insurance for care. Timing and eligibility depend on how and when services are requested or scheduled.

Without insurance

You do not have health insurance.

You may request a written estimate of the charges reasonably expected for your care.

Choosing self-pay

You have coverage but do not plan to use it.

Appropriate paperwork is required when you choose to opt out of using your insurance benefits.

Before care

You request an estimate or schedule ahead.

A Good Faith Estimate is generally provided when requested or when qualifying care is scheduled at least three business days in advance.

Current self-pay rates

A useful starting point, not your formal estimate.

These are Acuity Counseling’s current standard rates. Your personalized estimate may reflect the clinician, service, anticipated frequency, and expected duration of care.

Your personalized estimate

Built around the care reasonably expected for you.

Your written estimate is more specific than a public rate list. It reflects the information available when it is prepared and may be updated if your needs or care plan change.

Expected services

The estimate identifies the care and charges reasonably expected based on the information available at that time.

Anticipated course of care

Frequency and duration may be included when they can be reasonably anticipated.

Secure delivery

Acuity sends the personalized written estimate through the secure client portal for you to review and keep.

Your federal rights

If a bill is substantially higher than your estimate.

If a provider’s billed charges are at least $400 more than the amount listed on your Good Faith Estimate, you may be able to use the federal patient-provider dispute resolution process.

Keep a copy of your estimate. The Centers for Medicare & Medicaid Services explains eligibility, deadlines, fees, and how to begin a dispute.
